Commitment
I understand that full commitment to participating in the process and carrying out the follow up process needed for this therapy to be fully effective and that anything less than intentional full participation will not lead to success.
I accept full responsibility for myself and any actions I take that might result from receiving this therapy.
Confidentiality.
All communication and information transfer resulting from this agreement is to be regarded as strictly confidential on both sides. Records of the session will be retained securely offline for a period of 24 months as required by the professional standards body. Copies of your transformation recording will be retained at your request in secure storage for longer as a backup in case you lose your copy. The GDPR Privacy Policy as published on the Life Mastery Now website applies.
Appointments
Rescheduling of appointments must be done a minimum of 24 hours before the planned date and time. Failure to attend a scheduled session without the above notice will result in an additional fee of £25.
Medical Disclaimer
Please note, hypnosis alone is no guarantee of success. The client needs to desire the change and be open, willing and motivated to desire the change via hypnosis.
RTT is not intended as a replacement for conventional medical treatment. In certain circumstances we will contact your Doctor prior to the session as to the nature of any condition for which you are being treated. Do NOT make adjustments to ANY prescribed medication without the approval of your doctor. If in any doubt, you should seek your GP’s advice.
Technology (for online sessions)
For online sessions Zoom will be used and you will need a robust broadband connection with an absolute minimum speed of 3 mbps. The web address to link to the Zoom session is https://zoom.us/j/9755205874 . Please attempt to connect at least 10 minutes in advance to ensure that the technology is working. For some computers this may require a download of software from Zoom.
The computer being used must incorporate a webcam and microphone or be connected to appropriate external equipment.
You should test and ensure that everything is working properly and in accordance with these instructions at least 24 hours before the session and inform the Therapist immediately of any problems in case alternative arrangements need to be made.
Please note it is not practicable to attempt the Zoom session using a smartphone.
It is essential for the purposes of the session for the Therapist to be able to see your head, face, eyes and upper body including arms outstretched. The camera and lighting must be placed accordingly.
For all sessions, delivered face to face or online, I Agree:
- That I am not suffering from any diagnosed psychiatric condition, psychological illness or epilepsy and are not under the supervision of a psychiatrist.
- That I have provided my correct address and contact details.
- That I will take part in all sessions free from the influence of either alcohol or drugs and will provide a list of any prescription medicines I am currently taking.
- That I will ensure that the environment in which sessions are undertaken shall be safe and free from distractions and will inform the Therapist if there is or will be anyone else present or monitoring the session.
- That I will not record the session (either by sound or visual means) without prior permission from the Therapist.
- That all communications systems other than that being used for the session will be switched off.
- That I understand and accept that the Therapist may terminate the session without warning if they determine that the session has been booked for some purpose other than to receive therapy, and that should this occur, the client will remain liable for any session fee that may have been paid in advance.
- That I will provide a back-up number or text arrangement in case of technology failure, and a third-party emergency contact number.

EXPLANATORY NOTES
Technology
What if the internet connection drops?
If the internet connection drops at any point throughout your session, you will naturally come back to a normal, waking state of reality. At this point, you can simply reconnect and we will resume your session.
Communication
It is your responsibility to communicate with the therapist about any matters pertaining to the session in advance. You should expect to receive follow up emails and other communications regarding the results of the session.
Your Privacy
According to the GDPR rules applicable in the United Kingdom and Europe I need to tell you what data I am collecting from you and what I intend to do with it.

What data do I keep and why do I need it?
Name and age – this is basic information that helps me get to know you.
Address, email address, phone number – I use this as a way of contacting you regarding your sessions. I will mainly use the method you first contacted me on but if I cannot reach you, I will try a different method.
Next of kin/ GP’s details – If I was worried that you were at risk then I may need to contact your next of kin or GP, if I can. I will let you know when/if I am going to do this.
Session notes – I keep brief notes of our session(s) on paper, with identification data held separately,
Will I share your data and if I do, who will I share it with and for what purpose?
Your data will not be sold, shared or used for any unethical reasons. I may have to share it if required to by a Court or if you or anyone you tell me about is at harm or risk of harm I may have to pass this information on. I also have regular Supervision where I talk about my work but I only use your first name.
How will I store your data?
Notes are scanned and stored on a separate password protected hard drive in a locked cabinet and the original paper versions shredded. Your contact details are separated from your notes and stored in the same way.
How long will I store your data for and how will I dispose of it?
This information is retained for two years unless you formally request me to keep it for a longer period in case you wish me to refer back to it at a future date.
By booking on any programme with the Freedom Academy or with the Freedom Coach I affirm I have read and understand this agreement.
